For children 12 and under
- Two routine cleanings a year
- Two routine oral exams a year
- Fluoride treatment twice a year
- Necessary routine x-rays
- One emergency exam per year, if needed
- 15% off other regularly priced services paid by cash or check on the day of service
- 13% off other regularly priced services paid by credit card on the day of service
Adult
$465/year
For patients with regular dental cleanings (no periodontal disease)
- Two routine cleanings a year
- Two routine oral exams a year
- Necessary routine x-rays
- One emergency exam per year, if needed
- 15% off other regularly priced services paid by cash or check on the day of service
- 13% off other regularly priced services paid by credit card on the day of service
Periodontal
$875/year
For patients with periodontal (gum) disease
- Up to four periodontal maintenance cleanings, as your dentist determines
- Two routine oral exams a year by your general dentist
- Necessary routine x-rays
- One emergency exam per year, if needed
- 15% off other regularly priced services paid by cash or check on the day of service
- 13% off other regularly priced services paid by credit card on the day of service

How it works
- The program is not insurance. It is for people who have no insurance or choose not to use it.
- Membership runs one year from your enrollment date.
- Dues are paid in full at enrollment and are non-refundable. Cash, check and credit card are accepted.
- Membership renews automatically on its anniversary unless we receive a written request to cancel 30 days before renewal.
- Additional services are your responsibility, and payment is due on the date of service to get the discount.
- Membership must be current to receive the discount. Fees for dental services may change at any time.
- Please give at least 48 hours' notice if you need to change a scheduled appointment.
- Benefits can't be transferred to another dental or specialty practice.
